Community Transfer of Care Hub Case Manager

We are seeking a skilled and compassionate professional to join our Transfer of Care Hub team as a Case Manager. In this role, you will play a vital part in supporting patients to return to their own homes following acute illness or injury.
About the RoleAs a Case Manager, you will be responsible for providing person-centred, holistic assessments to facilitate discharge from acute hospitals to the right discharge pathway. You will work collaboratively with a multi-disciplinary team to identify the most appropriate discharge pathway for each patient.
You will also be responsible for triaging referrals received by the Transfer of Care Hub, liaising with patients, family members, and ward staff to identify the most suitable discharge pathway.
Key Responsibilities- Provide person-centred, holistic assessments to facilitate discharge from acute hospitals to the right discharge pathway.
- Triage referrals received by the Transfer of Care Hub and liaise with patients, family members, and ward staff to identify the most suitable discharge pathway.
- Work collaboratively with a multi-disciplinary team to identify the most appropriate discharge pathway for each patient.
- Utilise multi-disciplinary assessment skills, supported by sound clinical reasoning, to facilitate timely hospital discharge to the most appropriate environment.
- Effectively triage and prioritise a large caseload and adjust daily work plans in response to system-wide pressures and escalation.
- Pro-actively optimise the usage of community capacity on a daily basis.
