Outcomes First Group is seeking an experienced Team Leader to join our Adult Care team in Scunthorpe. As a Team Leader - Nights, you will be responsible for leading a team of Support Workers and ensuring the highest standards of care are delivered to our service users.
Key Responsibilities:
- Lead a team of Support Workers to deliver high-quality care and support to service users
- Develop and implement care plans that meet the individual needs of service users
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
- Collaborate with other teams to ensure seamless care delivery
Requirements:
- Experience as a Support Worker, Deputy Team Leader, or Team Leader in Adult Care
