We are seeking a highly motivated and experienced Mental Health Practitioner to join our dynamic team at North Staffordshire Combined Healthcare Trust. As a key member of our community mental health team, you will play a vital role in providing high-quality care and support to individuals with mental health needs.
Key Responsibilities- Conduct comprehensive assessments and triage referrals to ensure timely and effective interventions.
- Work collaboratively with a multidisciplinary team to develop and implement individualized care plans.
- Provide duty support and interventions to service users, including home visits and follow-up appointments.
- Liaise with other professionals to ensure high-quality, gold-standard care and support.
We are looking for a qualified and experienced Mental Health Nurse, Occupational Therapist, or Social Worker who possesses:
- A minimum diploma level or equivalent qualification.
- Experience of working therapeutically in community settings.
- Knowledge of various approaches to mental health assessment.
