About The Role

As a Senior Practitioner at the British Red Cross, you will play a vital role in coordinating care transitions, mentoring staff, and upholding the highest standards of service delivery. Your expertise will ensure optimal staffing levels and adapt work plans to meet evolving user needs.

Key Responsibilities:
  • Maximise independence through coordinating transitions from hospital to home and conducting ongoing care assessments.
  • Provide compassionate care in accordance with Care Inspectorate regulations, making a real difference in service users' lives.
  • Collaborate with the team to maintain optimal staffing levels and adapt work plans to evolving needs.
  • Empower individuals by connecting them to additional resources and services, while also mentoring and supporting fellow team members.

Requirements:
  • A SVQ Level in Health and Social Care (or equivalent) (preferred).
  • PDA in Supervision (preferred).
  • Educated to N level (or equivalent by experience).
  • Willingness to work flexibly in accordance with the pattern set out in the duty rotas in place.
  • Ability to accept responsibility for the overall running of the service in the absence of a senior member of staff.
  • Full UK Driving License is required and access to a vehicle. Mileage paid.
