Job Title: Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Four Seasons Health Care. As a key member of our nursing team, you will play a vital role in delivering exceptional care to our residents.

About the Role

As a Nurse Practitioner, you will be responsible for leading a team of nurses and carers to provide comprehensive care to our residents. You will work closely with our multidisciplinary team to develop and implement care plans that meet the unique needs of each individual.

Key Responsibilities
  • Lead a team of nurses and carers to deliver high-quality care to residents
  • Develop and implement care plans that meet the needs of each resident
  • Provide training and workshops to the team on best practices and new developments in care
  • Collaborate with the therapy team to develop and implement individualized care plans
  • Pre-assess potential residents and develop step-by-step discharge plans
  • Organize multidisciplinary team meetings to discuss resident care and progress
  • Advise residents on illness prevention, health promotion, and lifestyle choices
  • Provide training to residents and their families on health-related topics
  • Lead a team of care staff to deliver exceptional care and support to residents
  • Maintain accurate documentation and resident records, incorporating modern technology
  • Oversee all aspects of medicine management on your shift, adhering to company policies and legislation
Requirements

To succeed in this role, you will need:

  • Experience in brain injury care
  • Registration with the NMC and ability to lead a team of care staff
  • Passion for delivering high-quality care and supporting residents and their families
  • Excellent communication and interpersonal skills
  • Able to work effectively in a clinical environment
  • Awareness and understanding of relevant legislation and national care standards
