About the Role

We are seeking caring and compassionate individuals to join our team as Care Professionals. As a Care Professional, you will work in partnership with other health professionals and agencies to meet the needs of our residents. You will have access to their support plan, which will provide you with all the information you need to do the job.

Main Responsibilities

  • Promote patient-focused care
  • Assist residents with activities of daily living, promoting their comfort
  • Deliver support plans and safeguard vulnerable people
  • Contribute to outcome-focused reviews
  • Encourage residents' independence, choice, health, and wellbeing
  • Encourage individuals to use equipment and assistive technology
  • Facilitate residents' transition to alternative services and support if needed
  • Engage in organisational learning and development
  • Provide excellent patient care and support
  • Record care and complete documentation and feedback

Requirements

  • Desire to care and look after residents
  • Previous personal or professional care experience (desirable)
