Job Title: Older Peoples Community Therapist

We are seeking an experienced Occupational Therapist to join our team at Cambridgeshire & Peterborough NHS Foundation Trust as an Older Peoples Community Therapist. This is an exciting opportunity to work within our Older Peoples Community Mental Health Services, providing specialist occupational therapy interventions to older people with mental health conditions.

About the Role

You will work closely with multidisciplinary teams to deliver high-quality occupational therapy services to individuals with complex mental health needs. Your responsibilities will include providing specialist occupational therapy interventions, undertaking the role of care co-ordinator, and working closely with other professionals to ensure seamless care coordination and optimal outcomes for service users.

Key Responsibilities
  • Provide specialist occupational therapy interventions underpinned by the Model of Human Occupation and the principles of recovery and enablement.
  • Apply a high level of understanding of the effects of cognitive impairment and disability on occupational performance and provide training and advice on lifestyle changes and adaptations to the service users social and physical environment.
  • Undertake the role of care co-ordinator for a defined clinical caseload of service users with complex mental health needs.
