Job Overview:

Sutton Health and Care is an innovative partnership of providers working across Sutton to deliver integrated services for the local community. As part of this partnership, we have the exciting opportunity to implement a service model for the provision of community services that truly integrates health and care.

Main Duties:

We are looking for an enthusiastic Occupational Therapist to work as part of the Sutton Health and Care at Home service. You will be part of the multidisciplinary team that helps to prevent admission from community, accelerate discharge from emergency departments, and provide supported discharge through bed and home-based intermediate care.

Key Responsibilities:

  • Conduct assessments and develop care plans to support patients in their homes and communities.
  • Work collaboratively with multidisciplinary teams to deliver high-quality, patient-centered care.
  • Provide training and education to support workers and other healthcare professionals.
  • Participate in clinical audit and quality improvement initiatives.

Person Specification:

We are looking for an Occupational Therapist with significant relevant post-registration experience, postgraduate experience covering core rotations, and an appreciation of constraints to working in the community. You should have experience of working with support workers, conducting appraisals for junior staff, and participating in clinical audit.
