We are seeking a highly motivated and experienced Physiotherapist to join our team at North Care Network, a Primary Care Network (PCN) in the NHS. The successful candidate will be responsible for delivering high-quality rehabilitation services to patients, working closely with our multidisciplinary team to provide comprehensive care.
Main Responsibilities- Rehabilitation Services: Deliver programmes of supported patient self-management, facilitating behavioural change, optimising physical activity, mobility, and independence, and minimising the need for pharmacological interventions.
- Clinical Decision Making: Accountable for decisions and actions via HCPC registration, supported by a professional culture of peer networking/review and engagement in evidence-based practice.
- Communication: Communicate effectively and appropriately with patients and carers regarding diagnosis, pathology, and prognosis.
- Clinical Skills: Utilise a range of clinical skills, including non-medical independent prescribing, joint/soft tissue injections, and joint aspirations.
- Team Development: Provide learning opportunities for the whole multi-professional team within primary care.
- Complex Situations: Manage interactions in complex situations, including with individuals with particular psychosocial and mental health needs and with colleagues across the primary care team, sectors, and settings.
- Qualifications: Diploma/Degree in Physiotherapy, current full driving licence, post-graduate qualifications relevant to MSK & Orthopaedic speciality to Masters level or equivalent.
- Skills and Abilities: IT skills and ability to use computer packages, ability to reflect and critically appraise own performance and that of others, ability to identify need for service development and implementation of action plans to address.
