Key Responsibilities- Conduct comprehensive assessments and reviews for adults across the intermediate care pathway, ensuring high-quality, person-centered care that upholds the dignity and rights of service users.
- Work collaboratively across various settings, including inpatient units, acute hospitals, and within the community, to deliver seamless and effective care delivery.
- Participate in rotational duties, including working in rehabilitation units during periods of high demand, particularly around discharge planning.
- Develop and implement care plans that meet the unique needs of each service user, working closely with multidisciplinary teams to ensure effective care delivery.
- Qualified Social Worker with a valid registration with Social Work England.
- Strong understanding of the Care Act and experience in undertaking comprehensive assessments and reviews.
- Ability to work flexibly across different care settings, adapting to the needs of the service.
- Excellent communication skills, with a commitment to collaborative working and supporting positive outcomes for service users.
- Competitive pay rate of up to £36 per hour.
