About the Role

This role will provide unplanned nursing care for patients across East Exeter and Mid Devon out of hours, working as part of a team to deliver high-quality care and support.

Key Responsibilities
  • Deliver nursing care to patients in their own homes, supporting them to remain in their preferred place of care and preventing unplanned admissions.
  • Work collaboratively with the Out of Hours Nursing Team to provide a comprehensive service, including end of life care, admission avoidance, and support for timely hospital discharge.
  • Develop and maintain strong relationships with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in ongoing training and development to enhance clinical skills and knowledge, with support from Band 6 nurses, the Team Manager, and Clinical Matron.
  • Contribute to the development of the team, sharing best practices and ideas to improve patient care and outcomes.
Requirements
  • Experience in community nursing is desirable, but not essential, as full training and support will be provided.
  • Ability to work flexibly, including evenings and weekends, to meet the needs of patients and the service.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Commitment to delivering high-quality care and support, with a focus on patient-centered practice.
