Nurse Practitioner

This role involves providing unplanned nursing care to patients across East Exeter and Mid Devon out of hours. As a key member of the Royal Devon University Healthcare NHS Foundation Trust, you will be part of an exceptional team working together to help patients stay healthy when they need it most.
Main Responsibilities
As a skilled nurse, you will be responsible for providing high-quality nursing care to patients in their own homes, supporting them to remain in their preferred place of care and preventing unplanned admissions. You will work closely with the Out of Hours Nursing Team, consisting of nurses and healthcare assistants, to cover a broad range of cases, including end of life care, admission avoidance, support for timely hospital discharge, and urgent unplanned nursing needs.
Key Skills and Qualities
We are looking for a passionate and adaptable individual who is able to think on their feet and outside the box. You will need to demonstrate your enthusiasm and commitment to community nursing, as well as your ability to develop your clinical skills in meeting the varied needs of patients out of hours. The post-holder will be supported by Band 6 nurses, the Team Manager, and Clinical Matron, and will work with an HCA on all clinical visits.
