Key Responsibilities
  • Provide support and information to clients, helping them develop independent lifestyles through connection with supportive groups and activities.
  • Record and report client activities, ensuring appropriate actions are taken based on changes in their condition or circumstances.
  • Liaise with primary care, social care, health, housing, and other professionals to ensure comprehensive support for clients.
  • Collaborate with clients to establish short, medium, and long-term goals, developing personalized care and support plans.
Requirements
  • Demonstrate passion for achieving agreed outcomes and improving client well-being.
  • Work effectively under pressure, managing tasks with minimal supervision.
  • Strong team player, capable of collaborating within a multi-level team structure.
