GP Link Worker

We are seeking a dedicated and compassionate GP Link Worker to join our team. As a key member of our community-based health and social care team, you will play a vital role in supporting individuals facing various challenges and promoting their overall well-being.
Key Responsibilities- Provide timely support and guidance to individuals in need, working closely with community-based health and social care providers.
- Support individuals transitioning from hospital settings back into the community, focusing on promoting independent living and reducing the risk of readmission.
- Liaise with primary care, social care, health, housing, and other professionals to ensure comprehensive support for clients.
- Collaborate with clients to establish short, medium, and long-term goals, developing personalised care and support plans.
- Demonstrate a passion for achieving agreed outcomes and improving client well-being.
- Work effectively under pressure, managing tasks with minimal supervision.
- Be a strong team player, capable of collaborating within a multi-level team structure.
- Demonstrate accountability and professionalism in interactions with service users, partners, and stakeholders.
