We are seeking an experienced and highly skilled Primary Care Network Clinical Practitioner to join our dynamic team. This exciting opportunity will see you working closely with patients, clinicians, and other healthcare professionals to deliver high-quality, proactive care in the community.

Main Responsibilities

The successful candidate will be responsible for assessing, diagnosing, planning, implementing, and evaluating treatment/interventions and care for patients with complex needs. You will work autonomously within primary care, utilizing your enhanced clinical assessment and treatment skills to provide ongoing and coordinated care.

As a key member of our team, you will have a significant impact on the lives of our patients, making sound judgments in the absence of full information and managing varying degrees of risk when there is complex, competing or ambiguous information or uncertainty.

You will work closely with the wider multi-disciplinary team to deliver joined-up care, referring to other specialist providers and services when necessary.

Key Duties and Responsibilities:
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
  • Perform specialist health checks and reviews.
  • Undertake the collection of pathological specimens including intravenous blood samples, swabs, etc.
  • Be responsible for a list of vulnerable patients at your practice.
  • Once accustomed to primary care, assess and triage patients, including same day triage, and as appropriate provide definitive treatment or make necessary referrals to other members of the PCN team.
  • Maintain accurate and contemporaneous health records appropriate to the consultation.
