About the Role

We are looking for a highly skilled and motivated Occupational Therapist to join our team in a part-time capacity. As a member of the admission avoidance team, you will work closely with patients and their families to identify risks and develop effective intervention plans.

Key Responsibilities:

  • Assessing patients' needs and developing personalized intervention plans
  • Working collaboratively with multidisciplinary teams to ensure comprehensive care
  • Providing emotional support and education to patients and their families
  • Maintaining accurate records and contributing to quality improvement initiatives

Requirements:

  • Qualification in Occupational Therapy from a reputable institution
  • Recent experience working in the NHS, preferably in a community setting
  • Registration with the HCPC and a valid driving license
