**Job Summary:**

We are seeking a skilled and compassionate Support Service Coordinator to join our team in Bristol. As a key member of our supported living services, you will play a vital role in supporting individuals with learning disabilities, physical disabilities, autism, and mental health needs.

**Salary:** £11.75 per hour

**Job Description:**

As a Support Service Coordinator, your primary responsibilities will include coordinating care and support services, monitoring progress, and making recommendations to ensure optimal outcomes for individuals. You will work closely with colleagues to develop and implement personalized care plans, ensuring that each individual receives tailored support to meet their unique needs.
