Mental Health Specialist Coordinator

Salary: £48,821 - £60,451 per annum (dependent on experience)
We are seeking a highly skilled and experienced Mental Health Specialist Coordinator to join our team at Pennine Care NHS Foundation Trust. As a key member of our Crisis Care Service, you will play a crucial role in ensuring the safe and effective pathways for young people across Greater Manchester into specialist inpatient settings.
The successful candidate will have a relevant core professional qualification in mental health or social care, with substantial experience of risk assessment and risk management. You will be able to exercise clinical responsibility for patients' care and treatment and provide case management for young people with complex needs.
You will work closely with our inpatient colleagues and have the responsibility of managing the Single Point of Access for all Inpatient referrals for the GM population of 13-18 year old young people who are being considered for inpatient care. This will involve working with CYP with a variety of mental health difficulties and diagnoses such as Psychosis, Bipolar Disorder, Emotional dysregulation, Schizophrenia, Depression/anxiety, LD, ASD, ADHD, PTSD, Conduct disorder, and Attachment difficulties.
You will also have the opportunity to work closely with external services such as social care, CAMHS, FCAMHS, CMHT, CEDS, EIT, HTT to ensure the correct pathways are in place to support our CYP's discharge from the hospital.

Your Key Responsibilities
- Lead and co-ordinate an effective service to ensure there are safe and effective pathways in place for all young people across the footprint into specialised inpatient settings within the NHS and independent sector.
- Ensure the provision of a 5-day, high-quality clinical service to young people before, during, and after admission to a specialist inpatient setting, co-ordinating bed management for the whole of the NHS Greater Manchester GAU and ED beds.
- Promote and maintain partnerships and interface with other professionals and agencies that work within the child and adolescent mental health settings.
- Provide case management for young people when their pathways have become disrupted or have complex needs.
Your Qualifications and Experience
- Rélevant core professional qualification in mental health or social care plus evidence of continued professional development in a CAMHS related area.
- Substantial experience of risk assessment and risk management.
- Experience of assessment, formulation, and managing complex cases and risk within a mental health service.
- An extensive range of clinical experience within the specialty or within a care pathway.
- Experience of exercising clinical responsibility for patients' care and treatment and case management.
