Job Description

Job Summary

Millstream Medical Centre is seeking a highly skilled and dedicated Practice Nurse to join our team. As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the overall success of our medical centre.

Key Responsibilities

  • Implement and evaluate individual treatment plans for chronic disease patients, delivering patient care as necessary and referring patients to secondary/specialist care as required.
  • Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on patients' health.
  • Provide routine nursing care to patients as required, in accordance with clinical-based evidence, NICE, and the NSF.
  • Provide wound care to patients and request pathology services as necessary.
  • Maintain accurate clinical records in conjunction with extant legislation and ensure read codes are used effectively.
  • Maintain chronic disease registers and develop, implement, and embed well-woman clinics.
  • Chaperone patients where necessary and support the team in dealing with clinical emergencies.
  • Recognize, assess, and refer patients presenting with mental health needs and implement vaccination programs for adults and children.
  • Liaise with external services/agencies to ensure patients are supported appropriately and delegate clinical responsibilities appropriately, ensuring safe practice and the task is within the scope of practice of the individual.
  • Support the clinical team with all safeguarding matters in accordance with local and national policies and understand practice and local policies for substance abuse and addictive behavior, referring patients appropriately.
  • Deliver opportunistic health promotion where appropriate and participate in local initiatives to enhance service delivery and patient care.

Secondary Responsibilities

  • Monitor and ensure the safe storage, rotation, and disposal of medicaments.
  • Support members of the nursing/clinical team and participate in shared learning within the practice.
  • Continually review clinical practices, responding to national policies and initiatives where appropriate and participate in the review of significant and near-miss events applying a structured approach, root cause analysis (RCA).

Person Specification

Essential

  • Experience of working to achieve standards within the Quality and Outcome Framework (QoF) and working autonomously in a Practice Nurse role.
  • Registered with a Professional body and relevant Nursing/health degree and experience working in General Practice/Primary Care setting.
