About the Role
We are seeking a highly skilled and motivated Practice Nurse to join our team at Millstream Medical Centre. As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the delivery of our practice's mission to provide patient-centered care.
Key Responsibilities
- Implement and evaluate individual treatment plans for chronic disease patients, delivering patient care as necessary and referring patients to secondary/specialist care as required.
- Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on patients' health.
- Provide routine nursing care to patients as required, in accordance with clinical-based evidence, NICE, and the NSF.
- Provide wound care to patients and request pathology services as necessary.
- Maintain accurate clinical records in conjunction with extant legislation and ensure read codes are used effectively.
- Develop, implement, and embed well-woman clinics and support the team in dealing with clinical emergencies.
- Recognize, assess, and refer patients presenting with mental health needs and implement vaccination programs for adults and children.
- Liaise with external services/agencies to ensure patients are supported appropriately and delegate clinical responsibilities appropriately, ensuring safe practice and the task is within the scope of practice of the individual.
- Support the clinical team with all safeguarding matters in accordance with local and national policies and understand practice and local policies for substance abuse and addictive behavior, referring patients appropriately.
- Deliver opportunistic health promotion where appropriate and participate in local initiatives to enhance service delivery and patient care.
