Key Responsibilities

The ideal candidate will have experience in working in primary care, with a strong background in clinical pharmacy services.

This full-time position offers the opportunity to work in a collaborative environment, driving forward the development of clinical pharmacy services in primary care.

Responsibilities include leading medication reviews for patients with polypharmacy, managing aspects of the Quality and Outcomes Framework, and contributing to the overall success of the PCN.

Salary: £44 per hour, dependent on experience

Working Pattern: Full-time, Flexible Working, Home or Remote Working
