Exciting Opportunity for a Children's Community Nurse

We are seeking a highly motivated and proactive Children's Community Nurse to join our friendly and progressive team in Hillingdon. As a key member of our community children's nursing team, you will provide clinical nursing care, training, and support to children, young people, and their carers at home or in other community settings.

About the Role

As a Children's Community Nurse, you will be responsible for:

  • Providing skilled, evidence-based nursing care to children and young people with acute and long-term conditions
  • Working with patients and families to assess risks and needs, and develop care plans
  • Acting as a patient advocate in the multi-disciplinary team
  • Communicating effectively with all members of the team
  • Contributing to the creation and maintenance of high-performing teams
