We are seeking a dedicated and compassionate Support Worker to join our team at our 15-bed service in Chislehurst, providing specialist residential rehabilitation for adults with acquired brain injury or long-term neurological conditions.
About the Role:
- Provide high-quality, individualized care to residents, relatives, and carers
- Work collaboratively as a key member of the team to support residents in achieving their planned outcomes
- Build open and two-way relationships with residents, promoting their independence and well-being
- Support residents in their rehabilitation program, including activities of daily living and personal care
- Communicate effectively with the clinical team, reporting any issues or concerns
- Accurately record work activities and maintain accurate records
