We are seeking a dedicated and compassionate Support Worker to join our team at Active Care Group. As a Support Worker, you will play a vital role in providing high-quality care to our residents with acquired brain injuries or long-term neurological conditions.

What You'll Be Doing:

  • Providing individualized care to residents, relatives, and carers
  • Working as a key member of the team to support residents in achieving their planned outcomes
  • Building open and two-way relationships with residents
  • Supporting residents in their rehabilitation program
  • Establishing effective communication methods with residents
  • Supporting with activities of daily living and personal care
  • Communicating any issues to the clinical team
  • Accurately reporting and recording work activities
