Job Overview

We are seeking a highly motivated and enthusiastic Band 3 Reablement Assistant to join our team at St Helier Hospital in Carshalton. As a key member of our Multi-Disciplinary Team, you will play a vital role in delivering high-quality patient care and rehabilitation, enabling patients to achieve their therapy goals and promote independence prior to discharge home.

Main Duties of the Job

The successful applicant will work alongside physiotherapists, occupational therapists, nurses, pharmacists, and a GP to deliver patient-centered care. Key responsibilities will include carrying out therapy plans as per OT/PT recommendations, working as part of the MDT to ensure patients' safe and timely discharge home, and demonstrating a strong understanding of the role of a rehabilitation assistant.

Key Skills and Qualifications

  • Strong communication skills
  • Experience of working autonomously and as part of a team
  • Background experience within health or social care
  • Interest in rehabilitation

Person Specification

Education/Qualification

  • NVQ Level 3 health-related study or equivalent experience
  • Written, verbal, and numeric skills to GCSE or similar level

Experience

  • Experience of a caring role
  • Experience of working as a member of a team within health or social care
  • Experience in patient documentation

Skills/Abilities/Knowledge

  • Demonstrate ability and willingness to learn new skills
  • Basic IT skills, e.g., WORD, Outlook, Emails
  • Demonstrate understanding of the role of a rehabilitation assistant
