About the Role

The NHS is seeking an experienced and skilled Clinical Rehabilitation Specialist to lead our therapy input team at Glan Irfon Reablement/Rehabilitation Unit. As a key member of our team, you will be responsible for delivering high-quality patient care and developing the Discharge to Recover then Assess (D2RA) model.

Main Responsibilities

  • Admit and discharge individuals, providing training and development to staff">
  • Assess, intervene, and delegate rehabilitation plans">
  • Work as an autonomous practitioner, managing patients with complex needs in various care settings">

Requirements

  • Diploma/Degree in Occupational Therapy/Physiotherapy">
  • HCPC registration">
  • Significant postgraduate experience relevant to specialty">
  • Leadership skills and ability to manage a complex caseload">
  • Excellent communication and teaching skills">
  • Ability to travel and work in various sites based on need">
