Job Summary
This role is part of the Dorset Clinical Commissioning Group's efforts to provide high-quality care to older adults in the community. As a Community Mental Health Specialist, you will work closely with patients, families, and healthcare professionals to deliver comprehensive care and support.
Main Responsibilities
- Contribute to the assessment and development of care plans for older adults with mental health needs.
- Collaborate with multidisciplinary teams to provide therapeutic interventions and support.
- Work with patients and families to promote independence, self-management, and well-being.
- Develop and maintain relationships with community resources and services to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance care delivery and patient outcomes.
Requirements
- Education to degree or diploma level with a relevant professional qualification or equivalent experience.
- Demonstrable knowledge of assessment and therapeutic interventions in mental health.
- Experience in contributing to the management of change and implementing new initiatives.
- Ability to communicate effectively with patients, families, and healthcare professionals.
- Strong analytical and problem-solving skills to interpret complex situations and determine appropriate actions.
Desirable Criteria
- Recent experience in a comparable role within a community mental health setting.
- Experience in providing clinical supervision and mentoring to junior staff.
Personal Qualities
- Able to work independently and as part of a team to achieve shared goals.
- Strong initiative and ability to use own initiative within sphere of authority.
- Excellent communication and interpersonal skills to build relationships with patients, families, and healthcare professionals.
