Senior Clinical Care Manager

We are seeking a dedicated and experienced Senior Clinical Care Manager to join our team in the South London area.

Salary and BenefitsThe successful candidate will receive a competitive salary of up to £45,000 per annum (dependent on experience), plus a range of benefits including:
- 1 day working from home per week/4 days in the community
- Monday to Friday (9-5) hours
- 33 days holiday
- Private pension scheme
- Use of pool company car or travel card for community travel
- Business travel expenses of £0.45 per mile

- Company Mobile Phone and Laptop
As a Senior Clinical Care Manager, you will be responsible for providing face-to-face support to adult clients and their families, creating personalised care plans, and working closely with a core team of carers. Key responsibilities include:
- Designing and delivering tailored care plans specific to individual client needs
- Effectively managing the Multi-Disciplinary Team through strong communication and teamwork
- Providing ongoing monitoring, support, and assessment of clients' physical and physiological needs
- Delivering nursing interventions, including medication management
- Building and maintaining relationships with clients' families
- Working closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care
To be considered for this role, you must have:
- RGN with an active NMC PIN
- Supportive management style that brings out the best in others
- Up-to-date experience of tracheostomy care and ventilators within the last 2 years
- Advanced communication skills verbally and written to liaise effectively with patients, families, and the MDT
- Suitable for Band 5/Band 6 Nurses with evidence of effective leadership and management of HCAs
- Full UK Driving Licence desirable but not essential
