Job Title: Homecare Support Specialist

We are expanding our Intermediate Care Teams who deliver Discharge to Assess, Urgent Community Response, and Virtual wards in Dorset.

About the Role:

This is an exciting opportunity for a motivated and compassionate individual to join our supportive multidisciplinary teams as a Homecare Support Specialist. You will provide support to adult patients in their own homes and Care homes, promoting independence and improving quality of life by helping patients remain at home while being supported by a multi-disciplinary team.

The successful candidate will be responsible for providing admin support to ensure the efficient day-to-day running of the service, including providing rehab-based care to people in their own home, including a degree of personal care in the short term.

As a Homecare Support Specialist, you will work closely with other team members to monitor deteriorating patients with support from clinicians and contribute to the delivery of high-quality patient care across 7 days.

Responsibilities:
  • Providing rehab-based care to people in their own home, including a degree of personal care in the short term.
  • Monitoring deteriorating patients with support from clinicians.
  • Working collaboratively with other team members to achieve service goals.

The salary for this role is approximately £25,000 - £30,000 per annum, depending on experience, plus weekend enhanced pay.
