Client Care Coordinator

We are seeking a highly organized and compassionate care professional to join our team as a Client Care Coordinator. In this role, you will be responsible for coordinating care services and ensuring that clients receive the highest standard of care.

Responsibilities include:

  • Developing and implementing individualized care plans for clients
  • Coordinating care services with care providers to ensure seamless delivery
  • Communicating regularly with clients, families, and healthcare professionals to ensure ongoing support
  • Maintaining accurate records of client care plans and progress
Requirements

To succeed in this role, you will need:

  • A degree in Health & Social Care or a related field
  • Previous experience in care coordination, preferably in domiciliary care
  • Excellent communication and organizational skills to coordinate care services effectively
  • Knowledge of healthcare regulations and best practices to ensure compliance and quality care
