Job Summary

We are seeking a highly motivated and experienced Family Health Support Worker to join our team at Livewell Southwest. As a key member of our Public Health Locality Team, you will work closely with Health Visitors and School Nurses to deliver high-quality care and support to families with children aged 0-5 years.

Main Responsibilities
  • Plan and undertake home visits to deliver episodes of care, as delegated and assessed by the SCPHN.
  • Work in partnership with families, health professionals, and other agencies to provide comprehensive support and guidance.
  • Communicate effectively with clients, colleagues, and agencies within community settings and base.
  • Be competent to undertake delegated activities within the Healthy Child Program, utilizing appropriate tools and assessments.
  • Identify deviations in normal child development and refer to the SCPHN for further action.
  • Deliver and evaluate individual, clinic, and group activities in response to identified Public Health needs.
  • Implement identified packages of care that empower and meet client needs, reviewing effectiveness and modifying as necessary.

  • Essential:
    • Able to communicate effectively.
    • Able to work alone within a supervised framework.
    • Decision maker.
    • Enthusiasm for working with children and families.
    • Good listening and observational skills.
    • Able to travel across the city.
    • Available to work outside normal working hours, occasional evenings, and Saturdays for specific health promotion activities.
  • Desirable:
    • Works on an individual basis with children and families, supervised by the SCPHN, for time-limited, outcome-based episodes of care.
    • Recognizes areas of concern within families and follows agreed procedures.
    • Awareness of the importance of play for children.
    • Positive attitude.
