Job Title: Mental Health Support Worker

Join Livewell Southwest, an award-winning social enterprise providing integrated health and social care services, as a Mental Health Support Worker. This role is part of a key worker pilot designed in partnership with NHS England, expanding its current age range to 25 years old.

About the Role

This role will enable Children, Young People (CYP) and Young Adults and their family/carers to get the right support at the right time and ensure local systems are responsive to meeting their needs in a holistic and joined-up way. You will work across education, health, and social care to embed principles of transforming care and SEND reforms to improve the experience of CYP and Young Adults and families/carers.

Key Responsibilities
  • Active involvement in the delivery of high-quality, safe, and compassionate mental health services as an effective member of the multi-disciplinary team.
  • Implementing specific assessment, review, and care packages to individuals and groups within standard protocols, integrated care pathways.
  • Providing evidence-based advice and consultancy to young people, family members/carers within standard protocols.
  • Organising and managing a defined caseload under distant guidance and working as a lone worker where appropriate.
Requirements
  • Knowledge of a range of mental health presentations and medications.
  • An understanding of physical health problems which can impact on a person's mental health and monitoring of the same.
  • Knowledge of child protection and safeguarding procedures.
  • Basic IT skills.
  • Awareness of the Code of Conduct for Health Care Support Workers/Adult Social Care Workers in England.
Desirable
  • Understanding of relevant legislation pertaining to children's mental health.
  • Knowledge of child development.
