About the RoleThis is a full-time position, with 16 hours dedicated to administrative tasks and the remainder spent working in service. You will provide supervision and support to a team of Social Care Support Workers, ensuring they develop and sustain professional relationships with the people we support.
Key Responsibilities- Ensure staff promote self-esteem, happiness, and emotional health among the people we support.
- Encourage and support individuals in expressing their needs, views, and concerns.
- Enable people to make choices and decisions, and participate in planning and decision-making processes.
- Promote Person-Centred Planning and BILD principles.
- Respect and promote the rights and entitlements of individuals with learning disabilities, mental health, and autism.
- Ensure access to independent advocacy and advice.
- Support individuals in developing socially valued lifestyles, including culturally and age-appropriate experiences.
- Enable people to access social, leisure, work, and educational opportunities.
- Promote a warm and positive approach to friends and families of the people we support.
- Involve families and significant others in planning individual support, where preferred.
- Ensure individuals become as independent as possible, growing in confidence, competence, and personal effectiveness.
- Coach staff in using appropriate strategies and interventions to support individuals with challenging behavior.
- Act as a role model regarding authority, personal conflict, and responsibility.
- Ensure individuals receive necessary advice, care, and regular health checks to maintain physical and emotional wellbeing.
- Promote nutrition, relaxation, exercise, and a healthy lifestyle.
- Ensure medication is administered and recorded as prescribed.
- Organize safe procedures for collecting, storing, and administering medication within Autism Plus guidelines.
- Observe and monitor individuals' emotional and physical wellbeing, informing relevant staff and agencies of concerns or significant changes.
- Respond to emergencies and incidents promptly and appropriately, maintaining the welfare and safety of individuals and staff.
- Ensure staff carry out and record financial transactions involving service users within agency guidelines.
- Enable individuals to manage their personal finances independently.
- Ensure specific needs of individuals with additional problems are identified, assessed, and responded to as appropriate.
- Monitor and evaluate Individual Support Plans and person-centred plans.
- Ensure all individuals have a key worker and co-worker, and act as nominated key worker as appropriate.
- Work in homes, being visible to staff while maintaining privacy when dealing with confidential matters.
- Record staff rota hours, absences, and changes to working hours on Humanity, Workday, and the Main rota.
- Carry out back-to-work interviews and identify the need to progress outcomes to the next stage in line with policy.
- Lead and manage staff to achieve the highest levels of performance and standards of work, in line with PCP and CQC outcomes.
- Plan to meet the needs of service users as identified by their individual Person-Centred Plan.
- Coordinate and deploy staff resources efficiently in relation to the needs of service users and Autism Plus requirements.
- Observe safe working practices, including reference to Health and Safety regulations, lifting and handling, fire procedure, COSHH regulations, and Environmental Health requirements.
- Ensure all staff receive personal support, supervision, and appraisal.
- Take appropriate supportive and corrective action to address performance difficulties.
- Ensure staff training and development needs are identified and met.
- Participate in planning and delivering staff training and development activities.
- Coach, train, and supervise staff.
- Organize and chair team meetings, promoting a co-operative and cohesive team spirit and a culture of open and honest communication.
- Participate in promoting a team approach.
- Be polite, courteous, and supportive to all team members.
- Be flexible in accordance with the needs of the weekly rota.
- Be available to cover leave (sickness, annual, etc.).
- Cover on-call requirements in line with the coordinator's rota.
- Assist in the induction of new staff.
- Work in accordance with training and agreed policies, practices, and procedures.
- Participate in and contribute to staff meetings.
- Support colleagues in difficult or potentially difficult situations within the workplace.
- Elevate issues of concern to a more knowledgeable member of staff if the situation requires specific expertise.
- Minimum NVQ Level 2 in Social Care.
