About the Role

We are seeking a highly skilled and compassionate Care Manager Specialist to join our team at M&D Care. As a key member of our residential support services, you will play a vital role in delivering high-quality care and support to individuals with complex needs, including those with learning disabilities, autism, and mental health challenges.

Key Responsibilities
  • Lead a team of support workers in providing person-centred care and support to individuals in our residential settings.
  • Develop and implement care plans that promote independence, dignity, and well-being.
  • Work closely with our in-house specialist support services team to ensure seamless delivery of care and support.
  • Collaborate with healthcare professionals, local authorities, and family members to ensure comprehensive support for individuals.
  • Monitor and evaluate the effectiveness of care and support, making recommendations for improvement.
  • Act as an ambassador for our service, promoting our values and mission to stakeholders.
Requirements
  • QCF Level 3 in Health and Social Care or equivalent.
  • Experience working with individuals with learning disabilities, autism, and mental health challenges.
  • Strong leadership and management skills, with the ability to motivate and support a team.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with stakeholders.
  • Computer literacy and experience with Microsoft packages.
