About the Role

As an Advanced Care Practitioner, you will be responsible for assessing, diagnosing, treating, referring, and discharging patients with minor illnesses and within your level of competence and agreed protocols. You will also be required to work as an autonomous practitioner, providing first contact patient care and integrating clinical knowledge and skills into the out-of-hours service.

You will be working in partnership with medical, nursing, and allied health professionals to ensure delivery of a coordinated service. Your excellent interpersonal, verbal, and written communication skills will enable you to effectively communicate with patients, carers, and colleagues.

Key Responsibilities
  • Assess, diagnose, treat, refer, and discharge patients with minor illnesses and within your level of competence and agreed protocols.
  • Work as an autonomous practitioner, providing first contact patient care and integrating clinical knowledge and skills into the out-of-hours service.
  • Communicate effectively with patients, carers, and colleagues.
  • Develop and maintain an excellent learning environment, supporting clinical practice and development of knowledge/skills.
  • Contribute to quality improvement within the service through the identification and initiation of changes, which lead to better evidence-based practice and improved standards of care.
Person Specification

We are looking for a highly motivated and experienced Advanced Care Practitioner who possesses the following qualifications, skills, and qualities:

  • Registered General Nurse, Specialist Paramedic, or Clinical Pharmacist qualification.
  • Evidence of current NMC/HCPC/GPHC registration.
  • Non-Medical Prescribing qualification.
  • Excellent interpersonal, verbal, and written communication skills.
  • Able to demonstrate enthusiasm to developing clinical skills.
  • An understanding, acceptance, and adherence to the need for strict confidentiality.
  • Ability to use own judgment, resourcefulness, and common sense.

The salary for this role is £60,000-£80,000 per annum, depending on experience, plus benefits such as flexible working hours, DBS Enhanced Level Check, and opportunities for professional development and growth.
