About the Role

In this rewarding position as a Care Coordination Specialist, you will play a vital role in assessing and supporting individuals with diverse health conditions and disabilities. As a dedicated member of our team, you will be responsible for managing caseloads from start to finish, including face-to-face and virtual assessments, telephone interviews, initial review calls, and paper-based reports.

Key Responsibilities
  • Conduct thorough assessments of physical, sensory, mental, intellectual, and cognitive impairments.
  • Develop and implement personalized care plans that cater to the unique needs of each individual.
  • Collaborate with healthcare professionals and other stakeholders to ensure comprehensive support.
Requirements
  • Qualified Nurse or Mental Health Nurse with valid registration on the NMC without restrictions.
  • At least 1 year of post-qualification experience in a relevant field.
  • Excellent communication skills and strong IT literacy.

  • A base salary of £38,500 - £41,500, with opportunities for increase based on performance.
