Job Summary

CRA GROUP LIMITED is seeking a skilled Occupational Therapist Assistant to join our team in a 3-month ongoing contract with a local government organization. As a key member of our multidisciplinary team, you will be responsible for assessing and supporting vulnerable adults with physical disabilities, sensory impairments, mental health issues, and learning disabilities.

Key Responsibilities
  • Manage an independent caseload, assessing individuals with complex needs and developing personalized support plans.
  • Provide equipment, minor and major adaptations, and assistive technology to support independence and re-ablement.
  • Contribute to effective support planning, enabling citizens and carers to make informed decisions and exercise choice and control.
  • Facilitate assessments, feeding into specialist and complex assessments, in line with self-directed support and single assessment principles.
  • Support citizens to promote their health, well-being, choice, and control, visiting them in their homes and assessing their needs for equipment and adaptations.
  • Order and ensure timely delivery of equipment, reviewing provision with service users to ensure all needs are met.
  • Maintain comprehensive case records, assessments, and reports on service users.
  • Work closely with other team members, collaborating with colleagues from other disciplines and community agencies to enable people to access community resources and lead independent lives.
  • Participate in individual and team development activities, taking part in necessary training to perform the job effectively.
  • Promote and enforce the Council's Equal Opportunities and Health & Safety policies, working in accordance with adult social care and Walsall Council's aims and objectives.
Essential Experience and Qualifications
  • Demonstrate an understanding of the needs of vulnerable adults and their family/carers.
  • Demonstrate an understanding of personalization and self-directed support principles, enabling citizens to live independent lives and exercise choice and control in decision-making.
