Job Summary

We are seeking a motivated and enthusiastic Registered Nurse to join our Community Services Team at Woking & Sam Beare Hospices. As a key member of our team, you will play a vital role in delivering high-quality palliative care services to patients and their families in the comfort of their own homes.

Key Responsibilities

  • Provide expert nursing care to patients with life-limiting illnesses, working closely with our multidisciplinary team to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, identifying opportunities for improvement and implementing evidence-based interventions.
  • Collaborate with our therapy team, including occupational therapists, physiotherapists, and therapy assistants, to ensure seamless care coordination and delivery.
  • Develop and maintain strong relationships with patients, families, and carers, providing emotional support and guidance throughout the care journey.
  • Participate in regular team meetings and contribute to the development of new initiatives and projects to improve patient outcomes and staff satisfaction.

Requirements

  • Current registration with the Nursing and Midwifery Council (NMC)
  • Experience in palliative care, oncology, or respiratory nursing, or a willingness to learn and develop skills in these areas
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues at all levels
  • Ability to travel to patients' homes using your own vehicle, with a valid driver's license and car insurance
  • Flexibility to work a variety of shifts, including evenings and weekends, as required
