We are seeking a highly motivated and enthusiastic individual to join our team as a Community Learning Disability Nurse. As a key member of our multidisciplinary team, you will provide a phlebotomy, ECG, and physical healthcare service to patients within our community Adult Locality Team and the specialist Clozapine Clinic.
Main Duties- Provide a phlebotomy, ECG, and physical healthcare service to patients within the community Adult Locality Team and the specialist Clozapine Clinic.
- Work as part of a supportive multidisciplinary team that provides services to people who experience significant mental health challenges within a community setting.
- Adopt an enabling approach to help people achieve their recovery goals, promote independence, and develop or maintain their skills in daily living.
- Travel independently around the county, meeting strict time deadlines, and hold a full UK driving licence and have use of a vehicle.
