About the Role

We are seeking an experienced Proactive Ageing Well Specialist to join our team in delivering high-quality care and support to older adults within the community.

The successful candidate will work as part of a multidisciplinary team, using case-finding tools and pre-assessments to identify those who would benefit from proactive Comprehensive Geriatric Assessments (CGA), reducing unnecessary hospital admissions and enhancing the quality of life for older adults.

Key Responsibilities
  • Deliver high-quality care and support to older adults within the community, focusing on their unique needs and preferences.
  • Work collaboratively with the multidisciplinary team to develop and implement effective care plans, ensuring seamless coordination and communication.
  • Take the lead in advance care planning conversations, empowering patients and families to make informed decisions about their care.
  • Provide education and support to patients, families, and caregivers about managing conditions associated with ageing and available community resources.
Requirements
  • Registered healthcare professional with significant experience working in community settings, preferably with older adults.
  • Evidence of continuing professional development, including training in care of the older adult qualification.
  • Strong leadership and management skills, with ability to inspire and motivate the team to achieve excellent outcomes.

We offer a competitive salary of £58,000 - £68,000 per annum, dependent on experience, plus a range of benefits, including opportunities for career development and progression.
