Clinical Nurse Specialist

We are seeking a highly skilled and experienced Clinical Nurse Specialist to join our Urgent Community Response Service in East and West Sussex.
As a key member of our team, you will be responsible for providing clinical expertise and problem-solving support for our nursing team, as well as delivering high-quality care to patients in their own homes.
Main Responsibilities- To assess, plan, and provide clinical care needed for patients to be admitted to our Urgent Community Response Service.
- To work collaboratively with a multi-disciplinary team, including consultants, hospital staff, and social care professionals, to ensure seamless care delivery.
- To provide comprehensive and holistic assessments of patients in the community, identifying their needs and developing personalized care plans.
- To implement, monitor, and evaluate care programmes, ensuring that patients receive the best possible outcomes.
- To participate in the development of clinical care pathways and contribute to the improvement of our services.

Requirements- Experience of working in a community setting, with a strong understanding of care in the community.
- Ability to work flexibly, including evenings and weekends, to meet the needs of our service.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Knowledge of respiratory conditions, general surgical knowledge, and administration of intravenous therapy.
- Ability to develop and implement care programmes, and to evaluate their effectiveness.
