Job Title: Support Coordinator

About the Role

We are seeking a Support Coordinator to join our team at Cedar House in Newcastle upon Tyne. As a Support Coordinator, you will play a vital role in creating support plans with our customers and coordinating our small team of Support Workers to deliver high-quality care and support.

Key Responsibilities
  • Develop and implement individualized support plans to meet the unique needs of our customers.
  • Coordinate and lead a team of Support Workers to deliver care and support services.
  • Conduct risk assessments, support planning, and regular reviews to ensure the best possible outcomes for our customers.
  • Build strong relationships with customers, colleagues, and external partners to ensure seamless delivery of services.
