Job Title: Support Worker

Contract Type: Part-time, Permanent

Pay Rate: £11.44-£11.54 per hour

Location: PO7 7SR

As a Support Worker with Achieve Together, you will play a vital role in supporting individuals to live independently and achieve their goals. You will work collaboratively with our team to provide person-centered care and support, promoting the well-being and dignity of our service users.

Key Responsibilities:

  • Provide support and care to individuals in their homes and communities
  • Assist with daily living tasks, such as personal care and household chores
  • Encourage and support individuals to engage in activities and hobbies
  • Work collaboratively with our team to develop and implement personalized care plans
  • Monitor and report any changes in the individual's condition or well-being

Requirements:

  • Passion for delivering high-quality care and support
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to work a range of shifts, including evenings and weekends
  • Basic knowledge of healthcare and support principles
