**About the Job**
We are seeking a skilled Substance Use Case Manager to join our MHSU Intensive Case Management team in Cranbrook, BC.
This is a relief part-time opportunity until the return of the incumbent.
**Key Responsibilities**
- Provide assessment, treatment, service coordination, and urgent response to clients experiencing severe substance use concerns and concurrent disorders
- Function as a member of an interdisciplinary team-based model of care
- Develop and implement individualized treatment plans
- Collaborate with other healthcare professionals to ensure timely and responsive care
