About the Role

We are seeking a highly skilled and motivated Clinical Liaison Nurse to join our team at Southern Health NHS Foundation Trust. As a Liaison Nurse, you will play a vital role in providing high-quality care to patients with mental health needs in a general hospital setting.

Key Responsibilities
  • Develop and implement comprehensive care plans for patients with mental health needs
  • Work closely with multidisciplinary teams to ensure seamless care coordination
  • Provide specialist advice and support to hospital staff on managing psychological problems
  • Conduct risk assessments and develop safe management and discharge plans
  • Participate in educational sessions and quality improvement projects to enhance patient care

Person Specification

We are looking for a Registered Mental Health Nurse or Dual Field MH/Adult with excellent communication and interpersonal skills. You should have a strong understanding of mental health needs and be able to work autonomously in a fast-paced environment.
