Job Title: Senior Practitioner - Adult Community Services

Sanctuary Personnel Ltd, a leading recruitment agency, is seeking a highly skilled Senior Practitioner to join their Integrated Learning Disability Service in Enfield. This is a permanent, full-time position that offers a competitive salary and a range of benefits.

About the Role:

The successful candidate will work closely with the Social Work/Assessment & Care Management Services team to provide seamless health and social care services to individuals with learning disabilities and their families/carers. The ideal candidate will have a strong background in social work, with a minimum of four years post-qualifying practice, and experience in care management, safeguarding, and physical disability.

Key Responsibilities:
  • Conduct assessments and care planning in line with the Care Act, working collaboratively with partner organizations to achieve positive outcomes for service users.
  • Provide professional support and supervision to staff, negotiating case management tasks and assisting with supervisee's performance and personal development.
  • Work flexibly and imaginatively to manage complex situations, resolving issues and improving outcomes for service users and the organization.

Requirements:
  • A Social Work Qualification and registration with Social Work England.

  • Highly skilled professional competence in social work practice, with the ability to work within agreed standards of best practice and codes of conduct.
