Job Title: Street Healthcare Outreach Nurse

We are seeking a compassionate and skilled Street Healthcare Outreach Nurse to join our team at Change Grow Live. As a key member of our outreach team, you will provide high-quality clinical services to individuals experiencing homelessness, working closely with our service users to address their unique needs and challenges.

About the Role:
  • Provide clinical interventions and healthcare services to individuals experiencing homelessness
  • Work collaboratively with our outreach team to develop and implement care plans tailored to each service user's needs
  • Conduct assessments and provide guidance on healthcare and clinical interventions
  • Collaborate with local healthcare services and organizations to ensure seamless care coordination
  • Participate in ongoing professional development to stay up-to-date on best practices and evidence-based interventions

Requirements:
  • Registered Nurse with current registration and a valid NMC PIN
  • Experience working with vulnerable populations, including those experiencing homelessness
  • Strong clinical skills and ability to work independently
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively as part of a team
What We Offer:

We offer a competitive salary range of £43,814.50 - £48,183.48 (pro rata for part-time hours), a fixed-term contract until February 2026, and the opportunity to make a meaningful difference in the lives of our service users.
