Job Title: Street Healthcare Outreach Nurse

We are seeking a compassionate and skilled Street Healthcare Outreach Nurse to join our team at Change Grow Live. As a key member of our outreach team, you will provide high-quality clinical services to individuals experiencing homelessness, working closely with our service users to address their unique needs and challenges.

Key Responsibilities:
  • Provide clinical interventions and healthcare services to individuals experiencing homelessness
  • Work collaboratively with our outreach team to develop and implement care plans tailored to each service user's needs
  • Conduct assessments and provide guidance on health and wellbeing issues
  • Develop and maintain relationships with local healthcare providers and services to ensure seamless care coordination
  • Participate in training and professional development opportunities to enhance clinical skills and knowledge
Requirements:
  • Registered Nurse with current registration and a valid NMC PIN
  • Experience working with vulnerable populations, including those experiencing homelessness
  • Strong communication and interpersonal skills, with the ability to build rapport with service users and colleagues
  • Ability to work independently and as part of a team, with a flexible approach to working hours
  • Knowledge of local healthcare services and systems, with the ability to navigate complex care pathways
