About the Role

We are seeking a highly motivated and skilled Mental Health Practitioner to join our Adult Community Team in South Norfolk and surrounding areas.

The successful candidate will be responsible for assessing, planning, reviewing, and delivering care and treatment to service users on their caseload. They will also be expected to support duty cover for the team and liaise with other agencies to ensure care needs are met.

Key Responsibilities
  • Assess and develop care plans for service users
  • Deliver care and treatment to service users
  • Support duty cover for the team
  • Liaise with other agencies to ensure care needs are met
Requirements
  • Nursing degree or equivalent
  • Ongoing professional development working towards degree level
  • Experience in mental health practice
  • Effective risk assessment and risk management experience
