About the Role

We are seeking a highly skilled and experienced Senior Clinical Nurse Leader to join our Intensive Support Team as a Community Learning Disability Nurse Specialist.

The successful candidate will provide day-to-day clinical and operational leadership to an all-age intensive support team, delivering an enhanced model of support to vulnerable individuals at risk of hospital admission.

As a key member of our multi-disciplinary team, you will work closely with senior nurses and allied health professionals to reduce avoidable admissions to hospital and provide support for those in hospital.

Main Responsibilities

Key responsibilities include:

  • Providing clinical leadership and guidance to the team
  • Developing and implementing evidence-based clinical interventions
  • Collaborating with other teams to ensure seamless care transitions
  • Providing training and supervision to team members

Requirements

To be successful in this role, you will need:

  • Registration as a Learning Disability Nurse with the NMC
  • Accredited behavioural qualification at level 5 or above
  • Significant experience of working clinically with people with intellectual and developmental disabilities
  • Excellent communication and leadership skills
